MP Weight Loss provides the Patient Portal as a secure way to access certain health information, complete forms, review clinic information, and communicate with the clinic. By requesting and using Patient Portal access, I acknowledge the following:
- The Patient Portal is not for emergencies or urgent medical concerns. For a medical emergency, call 911 or seek emergency care.
- I am responsible for keeping my password, verification codes, passkeys, and other sign-in information private and for securing the phone, tablet, or computer I use to access the Patient Portal. I will log out when using a shared device.
- I will access only my own Patient Portal account, or the account of a minor or dependent for whom I am an authorized parent, guardian, or other authorized representative.
- Information I enter, forms I submit, and messages I send through the Patient Portal may become part of my medical record and may be collected, stored, reviewed, used, and disclosed by MP Weight Loss as permitted for treatment, payment, healthcare operations, care coordination, administrative purposes, and other lawful uses.
- Patient Portal messages and requests are not monitored continuously and may not receive an immediate response. I will contact the clinic directly when I need timely assistance.
- Electronic access to health information carries privacy and security risks, including risks from lost or stolen devices, compromised credentials, shared devices, or unauthorized access.
- I will notify MP Weight Loss promptly if I believe my Patient Portal account, device, password, verification method, or health information has been accessed or compromised without authorization.
- MP Weight Loss may restrict, suspend, or terminate Patient Portal access when reasonably necessary for privacy, security, legal, or administrative reasons.
- Patient Portal access does not replace appropriate medical care. I will contact MP Weight Loss or another healthcare professional with questions about my treatment and will not change prescribed treatment solely based on information displayed in the portal unless directed by my care team.
